WORKSHOP REGISTRATION

Registration fee includes: one copy of the conference proceeding, three breakfasts and lunches, one banquet, and one reception ticket.

Note, cancellation made before June 30, 1999 will receive 80% refund. No refund will be made after that date.

Registration procedure:

1. Submit registration information: Fill in the Workshop Registration Form and submit it. This will register your information and return a Confimation Page containing a reference ID number.
   

   You have the following payment options:

   • Credit card, submitting credit card information electronically
   • Credit card, submitting credit information by FAX or mail.
   • Check.

   IMPORTANT: You should always complete this step - also if you choose to fax or mail credit card information or pay by check.

2. Confirmation: Wait for an automatically generated web-page confirming the Registration (please be patient).
   This page also contains addrees and FAX information if you have chosen to pay by credit card (submitting card info by FAX or mail) or check.

NNSP rates (before July 26, 1999):

Concourse Level:	US$ 85.00/95.00 single/double occupancy per night.
Governor's Club:	US$ 125.00/135.00 single/double occupancy per night. Guests in the Governor's club (GC) receive complimentary Continental Breakfast in the morning, complimentary cocktails and hors d'oeuvres in the evening, turn down service and bathrobes in their room. All rooms in the GC have a single King bed.

Notice:

• These workshop group rates are applicable during August 19-27, 1999.
• 13.5% local tax and indidentals will be added.
• Additional person costs US$ 10 per night.
• Check-In time is 3:00pm, Check-Out time is 12:00 noon
• Upon Check-In, an imprint of your credit card OR a $10 per day deposit will be required (this is for possible incidental expenses).
• Cancellations must be recieved at least 48 hours prior to reservation date to receive a refund.
• To obtain special rates, reservations should be made by midnight July 26, 1999.
